Overview
EzEdMed is a platform service documentation system for direct services, indirect services (consultation), and assessment for students who are consistently seeing a mental health provider. This includes students with IEPs and 504s. It is also for general education students who are meeting with a mental health provider consistently and more than once.
Special Service Providers should be documenting session notes weekly in EzEdMed. Documentation of each service provides a) proof that a service was provided; b) data regarding the student’s performance; c) the possibility of reimbursement from Medicaid for those students who are Medicaid-eligible.
Guidelines
- Special Service Providers must complete an initial ezEdMed training through the School Medicaid Consortium (link below).
- Documentation of services is expected for any student on a health-related plan, including 504s, IEPs, Health Care Plans, Social-Emotional, and Mental Health (SE/MH Plans).
- Special Service Providers need to complete documentation weekly at a minimum. It is highly recommended that SSPs complete documentation daily if possible, in order to stay current.
- Special Service Managers will receive monthly notifications regarding the documentation completed by the staff in their department.
- Special Service Managers will send notice to any provider whose monthly report shows they have not been documenting services.
