Purpose: Staff who have opted in to FAMLI will need to submit a request form in Workday to self enroll.

If you plan to enroll in the State of Colorado's Family and Medical Leave Insurance (FAMLI) program, you must submit this form to log your participation. This notice enables DSST to provide information and assistance with wage submission, premium calculations, and the coordination of your other benefits if you need to take FAMLI leave.

Before submitting this request form, please look over this Leave Options Guidance form in order to better understand the FAMLI benefit and how it is different from FMLA and DSST Paid Family and Medical Leave.
Log into Workday and click on the Requests application from your menu.
Step 1: In the top left-hand box labeled "Actions" click on "Create Request"
Step 2: On the "Create Request" screen, select "All" in the drop down and then select "FAMLI Self Enrollment" and click “OK” at the bottom of the screen.
Step 3: On the next screen, answer the required questions.
Step 4: Click Submit at the bottom of the screen. If you have any questions about a submitted request, please send an email to humanresources@scienceandtech.org.
Step 5: Your request will be sent to your HR Partner for review. You will receive an email once your request has been processed regarding the next steps you need to take to complete this process.
