Step 3: In the “Request” section, click on “Request Return from Leave of Absence” to enter the date you are returning to work:

- If you are returning after the birth or adoption of a child, you MUST upload a copy of the birth certificate or discharge papers showing the child’s date of birth.
-
- ***Note: This must be completed within 30 days in order to open a Special Enrollment for benefits changes and elections (including adding a child to your insurance). See the user guide on Changing Your Benefits Outside Open Enrollment.
-
- If you are returning from leave due to your own medical condition early, you MUST upload a Fitness for Duty form completed by your medical provider.
- If you are returning to work at the scheduled time due to your own medical condition or that of a family member, please re-upload the original medical certification form used to request your leave.
- If you are returning to work after your original return date, reach out to your location's HR Partner.
Once your request is submitted, it will be reviewed by your HR Partner and either approved, or you will be contacted for additional information.
